IMPORTANT PATIENT NOTICE
COVID-19 (Coronavirus) - Updated 26th March 2020
Due to the Coronavirus outbreak, we are experiencing some operational issues and the health and safety of our patients and staff is of the utmost importance to us.
During the current Coronavirus (Covid-19) restrictions, we are asking all patients to avoid attending the Surgery wherever possible. We are still dispensing medication for collection from the surgery, but home delivery of your medication can be put in place if you are in the "high risk" group of patients, you live alone and are unable to get to the surgery and have no-one who can collect your medication for you or if you have been diagnosed or have definite symptoms or coronavirus.
We ask all patients to please use their SystmOne access or the NHS App for ordering of repeat prescriptions. Prescription requests are currently still being managed within our usual time frames, however, our Community Pharmacy colleagues are experiencing very high demand and we would ask that you give them additional time in which to prepare your prescription.
If your problem can wait until Covid-19 out break is over, then thank you for waiting and contact us once the outbreak is over.
We are aware that the demand for appointments and reassurance is increasing, as patients become more concerned. In order to manage the volume and to protect both ourselves and patients we are temporarily adjusting the appointment system.
All patients will now be triaged by a same day telephone call. GPs will have the option to still see the patient once they have assessed the case over the telephone.
Appointments with the Nursing team are being offered on a reduced basis.
If your problem can wait until the Covid-19 outbreak is over, then thank you for waiting and contact us once the outbreak is over.
We are attempting to reduce the footfall in the surgery and therefore reduce exposure to any patients or staff

STAY AT HOME AND STOP CORONAVIRUS SPREADING !!
Everyone MUST stay at home to help stop the spread of Covid-19.
This includes people of all ages, even if you do not have any symptoms or other health conditions.
Anyone can spread coronavirus. YOU MUST NOW:
- Only go out when absolutely necessary for food, medicine, essential work or exercise.
- Always stay 2 metres apart.
- DO NOT meet others outside your household, even friends and family.
We ask all patients requiring health advice and reassurance about COVID-19 to use the NHS website https://www.nhs.uk/conditions/coronavirus-covid-19/ for the most up to date guidance including isolation advice.
HOSPITAL OUTPATIENT APPOINTMENTS AND REFERRALS DURING COVID-19
In line with well-established plans, every hospital in England has now been asked to suspend all non-urgent elective operations and outpatient appointments, with some other procedures likely to be rescheduled to reduce spread and maximise the NHS's ability to manage the pandemic and meet the government's movement restrictions. People who are affected will be contacted by the relevant department as soon as possible.
Emergency cases and cancer treatments will be carrying on as normal, but we know many people waiting for treatment will be disappointed or worried. If you have been referred urgently to secondary care, including for suspected cancer, it is very important that you still attend these appointments. However, if you have symptoms of coronavirus or are self-isolating, please contact the relevant hospital department in advance via the telephone number on your appointment letter for their advice.

Bramham Medical Centre is a 3-partner, semi-rural practice situated in the village of Bramham located within the City of Leeds Metropolitan Borough.

The technology also means you can now do a lot of things from the comfort of your home such as order a repeat prescription, book or cancel an appointment online, if you are registered for this service. You can also use eReception below to book routine appointments or if you have an enquiry about your medication etc, and anyone registered at the practice can use this service.

PATIENT COMPLAINTS CLINIC
The surgery has decided to trial a "Complaints Clinic" to commence 3rd March 2020. The clinics will run for 3 months on a trial basis every Tuesday between 10am and 4pm on a by appointment ONLY basis. These appointments will be held by our Complaints Manager Mrs Pauline Tidswell and will be booked in 15 minute slots.
So..........Do you have a complaint/grumble regarding the surgery and would like to voice your concern without making a formal complaint OR do you want to make a formal complaint? Then please request an appointment, either by speaking to one of our Receptionists at the surgery, telephoning the surgery or email us on email@example.com and ask for an appointment with Mrs Tidswell in the 'Complaints Clinic', someone will then call you to arrange a date and time for your appointment.
We can only resolve/improve things if we know what the problems are!
DEMENTIA FRIENDLY AWARENESS SESSIONS
Following on from the successful Dementia Awareness Sessions held in September 2019. The Parish Council are hoping to extend the sessions into 2020. The sessions will be run with the help of a trained "dementia champion" from WiSE and in collaboration with the GP Surgery, who are fully behind the initiative. The Council are also hoping to extend the sessions to local businesses, the school and community groups in the future. The aim of these sessions is not only to improve the understanding of the condition in the village, but also to improve local access to support and services. EXTRA CLOSURE DAYS
As well as Public Bank Holidays, the Practice also closes from time to time for staff education and training, this is externally organised by the Leeds NHS Clinical Commissioning Group and is in line with all GP Practices within the Leeds CCG.
On the following afternoons the Surgery will be closed for staff training from 12:00 noon:

PATIENT PARTICIPATION GROUP (PPG)
Bramham Medical Centre PPG is a new partnership between patients of Bramham Medical Centre and the doctors, nursing team and administrative team who look after your wellbeing. The aim of this group is to encourage a positive working relationship between patients and surgery staff.

eReception is available 24 hours a day 7 days a week to enable patients to contact the practice. We can only reply to your enquiries between 8am and 5pm Monday - Friday. Outside these hours we will respond the next working day. Weekend submissions will be dealt with on Monday.

Please note: eReception should only be used for routine enquiries - please do NOT use eReception for urgent matters. If your matter is urgent please speak to reception directly either by phone or in person at the surgery.
Please also be aware that if you are wanting to book an appointment with a GP or a nurse, eReception is for ROUTINE appointments ONLY and these may be up to 4 weeks in advance.
Extended Access Service
There are now extra routine appointments available in the evening, making it easier for you to get an appointment at a time to suit you. There is also a virtual clinical Pharmacist service and a virtual Musculoskeletal (MSK) service, available 7 days a week including bank holidays. Due to the COVID-19 crisis, the online service is temporarily "out-of-service". All appointments are by telephone triage ONLY. You can still use eReception for routine queries.

You need to be registered for SystmOnline to use this service.
To register you will need to complete a "SystmOnline Application" form. Go to the Patient Resources page here where you will be able to print and complete the form.
Once registered, if you have a Smartphone, you can download the SystmOnline mobile app which is available for Apple and Android devices free of charge from the relevant app store.
