These appointments are with our nursing team, some of whom have specialist areas of interest and are able to amend treatment within their competence as they are Non Medical prescribers. Other members of the team undertaking the review will escalate concerns with the management of the condition to an ANP or GP. All team members undertaking reviews have been trained.
The aim of the review is to discover and tweak treatment and/or self management techniques to prevent major issues arising which can prevent exacerbations and hospital admissions.
We offer all patients with a long-term condition an annual review to help ensure that their self management and treatment for their condition is being optimised. These reviews include management of Asthma, COPD, Diabetes, Pre-Diabetes and heart disease, including Hypertension
