Alcohol & Drug Practitioner

We now have an Alcohol and Drug Practitioner from Forward Leeds who will be working out of Wetherby Surgery on Wednesday afternoons 12noon – 3pm from January 2025.

This is free and confidential, alcohol and drug support for adults, young people and their families for people in the Leeds area. The Practitioner will support you to help achieve your goals around your alcohol and/or drug issues through a range of treatments. That might be stopping completely or just cutting down. You can set your own goals. The Practitioner will do the initial assessment and the one-to-one work from the surgery weekly, this can be anything from brief interventions to referral into detox and rehab. The Practitioner will work with drugs users such as cannabis and cocaine etc, however, patients using heroin or opiates will be seen at the hubs within Leeds. The Practitioner will liaise with doctors when there is a need and will ask for bloods to be done at the surgery when referring into detox or prescribing of relapsed prevention medications.

To be referred to our Alcohol & Drug Practitioner if you feel you need help, just ask one of our Reception to team or speak with a GP and an appointment can be made for you.

The services offered coming into treatment include:-

  • One-to-one support
  • Structured group therapy work on a range of topics
  • Support for family members – involving them in recovery when appropriate
  • Health and wellbeing checks, health screenings, blood testing and vaccinations from our medical team
  • Support on reducing the harm of drugs and overdose prevention
  • Substitute medication for certain drugs
  • Detox within the community/at home or at a rehab centre
  • Peer mentors for additional support
  • Specialised support for those with complex needs such as housing, enduring mental health issues or pregnancy

CUES (COMMUNITY URGENT EYECARE SERVICE)

The Community Urgent Eyecare Service (CUES) provides urgent assessment, treatment or referral for sudden onset eye problems such as:-

  • Temporary and sudden loss ofe vision
  • Flashes and floaters
  • Minor eye injuries, pain and irritation
  • Eye infections that have not responded to treatment
  • Triachiasis
  • Double vision (Diplopia)

This service is open to adults and children registered with a GP practice in Leeds. Children under 16 who attend the service must be accompanied by a responsible adult. Access to this service is only available by calling 0300 303 4922. Following an initial consultation, an appointment may be arranged with a clinician, either virtually or face-to-face, if required. If you have dry eye or red eye symptoms you maybe advised to self-care or seek support from your local pharmacy first. Please note, this is not a sight test. Major eye conditions like cataracts, diabetic retinopathy, or glaucoma that are being regularly monitored by your optometrists or hospital eye service, will not be covered by this service.

What happens when you call

  • Call 0300 303 4922 free of charge, Monday to Saturday, 9am to 5pm. For
    out of hours, please call 111.
  • You’ll be asked some questions to make sure you are eligible to be
    treated under the service.
  • If eligible, you’ll be offered either a remote consultation over the phone or
    video call with an eyecare professional, or a face-to-face appointment at a
    local optical practice.
  • If not eligible, you’ll be given information about other options such as a
    community pharmacy, your GP practice, or an optician outside the service
    (there may be a charge for this).

f you are required to attend an appointment in person, please take your glasses and a list of your current medication with you to the appointment. The optometrist may put drops in your eyes to enlarge your pupils to get a better view inside your eyes. You should not drive until the effects of these drops have worn off, which may take a few hours. Appointments are typically available, where required, from 9am – 5pm, Monday –Saturday. Some practices may offer in-person appointments in the evenings and weekends. Not all practices have an optometrist available every day, but if they don’t, they will be able to find you an alternative appointment nearby.

Travel Vaccinations

Travel is a non-NHS service. If you are planning a trip abroad and think you require any vaccinations relating to foreign travel, please use the website Travel Health Pro to determine what vaccinations you are required/advised to have and Fit to Travel for up to date travel advice for your destination. This will include which countries and areas within the countries you are visiting.

It is up to you to decide what vaccinations to have.

You then need to make a 20 minute appointment with the practice nurse at least 2 weeks before you travel, to allow the vaccines to work.

When booking the appointment please inform the reception staff when and where you will be travelling, as some travel vaccines are not stock vaccine and have to be ordered in advance. Some travel vaccines have to be ordered on a private prescription and these incur a charge over and above the normal prescription charge. This is because not all travel vaccinations are included in the services provided by the NHS.

Free Travel Vaccinations Included on the NHS are:

  • Polio, Diphtheria and Tetanus booster (3 in 1 vaccine) – 10 years protection;
  • Hepatitis A – 25 years protection after 2 doses;
  • Typhoid fever – 3 years of protection;
  • Cholera (only if considered high risk).

All other travel vaccinations are treated as private for travel purposes and therefore command a fee. Therefore, if your Travel Health Pro information recommends other vaccinations that we cannot provide, you can obtain them from a private travel clinic, but please be advised, you will need to book an appointment with a private travel clinic at least 6 weeks in advance of your intended trip, as you may need a course of vaccinations.

Fitness to Work Certificates

You only need a Fitness to Work Certificate (sick note) if you are unable to work and are ill for longer than 7 calendar days. Your employer will provide you with an SC2 self-certificate form for shorter periods of illness. We can provide form SC1 for unemployed or self-employed patients.

If you do need a Fitness to Work Certificate (sick note) after the 7 day period, you will need to have seen or make an appointment to be seen by a GP, before a certificate will be issued.

Non-NHS Services

Some services provided are not covered under our contract with the NHS and therefore attract charges and these could take up to 40 days to complete. Examples include the following:

  • Medicals for pre-employment, sports and driving requirements – HGV, PSV, Taxi, Occ Health etc (by Face to Face appointment on Wednesdays ONLY)
  • Insurance claim/cancellation forms
  • Prescriptions for taking medication abroad.
  • Private sick notes and Private prescriptions
  • Vaccination certificates / Private immunisations / Private blood tests
  • Fitness to travel certificates.
  • Firearms Licencing & Medical forms
  • Court Exemption letters
  • Simple GP signature / stamp
  • Certificate completion

The fees charged are based on the British Medical Association (BMA) suggested scales and our reception staff will be happy to advise you about them.

It’s important to understand that GPs are self employed and offer their services to the NHS and they have to cover their costs eg staff, building, heating, lighting etc. The NHS cover these costs for NHS work, but not for non-NHS work.

Child Health & Immunisation

It is important that all pre-school children are fully immunised. All immunisations are performed by our specialist nurses in their routine clinics.

Exercise by Prescription

We are part of a scheme run by the Tadcaster Swimming Pool and Tadcaster Leisure Centre involving a programme of monitored exercise at a reduced cost. To use this service, you must make an appointment to see a GP.

Mental Health & Wellbeing Clinics

Access to these clinics is via referral from the GP or alternatively, for mild to moderate depression/anxiety you can self-refer here Mental Health & Wellbeing
