New Patient Registration

Our Practice Boundary

Before registering, please enter your postcode in the Search bar at the top right of the map, to ensure that you live within our service catchment area. Once you have entered your postcode, zoom out of the map to see if the postcode is within the boundary area.

Catchment Area

You do not need to make an appointment to register with the Practice. You will only need to make an appointment if you require repeat medication.

If you wish to register with the Practice, you can either pick up a registration form from Reception or download and print the forms below and bring into the surgery with your proof of ID.

You will need to download forms:- GMS1 Patient Registration form and the appropriate New Patient Health Questionnaire. And if you wish to use our online service, you will need to download the Patient SystmOnline Application form.

It is important that you complete all the fields on the registration form as an incomplete form may result in a delay with your registration. (You will need to contact your old surgery if you do not know your NHS Number).   

You will be invited at a later date to attend a brief consultation with a nurse to obtain some initial information about your health.

You have the right to express a preference for your registered doctor but it may not be possible for you to see this doctor each time you wish to be seen.
